Premiere Coaching Agreement

IMPORTANT-READ
This Agreement, is for Professional Life Coaching services with Susan A. Casserd. Life Coaching is a powerful, comprehensive process of personal healing, growth and transformation. The purpose of our interaction is to keep you on purpose and aligned with your intentions and to support you in realizing and activating those intentions.
Each session is scheduled at your convenience. If you need to reschedule, a minimum 24 hour advance notice is required except in the case of a medical, family, or work emergency; otherwise the session will be billed in full.
All information about you will be kept strictly confidential, with the exception of private and confidential consulting with another professional by me as needed if I feel it would be beneficial for you.
Your agreement and purchase is covered by a 30-day money-back guarantee, as long as you attended your session(s) and made a good-faith attempt to complete homework assignments.
The following describes your personal responsibility in our interactions:
1) As client, I understand and agree that I am fully responsible for my well-being, including my choices, interpretation of our discussions and decisions (if there is anything unclear to you, please ask for more clarity).
2) I recognize that life coaching is not psychotherapy and does not treat medical or mental disorders.
3) I understand that Professional Life Coaching is a comprehensive process that may involve all areas of my life. I acknowledge that deciding how to handle these issues and implement my choices is exclusively my responsibility.
4) As client, if I am currently under the care of a medical professional or mental health professional, I may wish to make this person(s) aware of my decision to enter a coaching partnership.
5) I understand that Susan A. Casserd is not a medical professional and does not give advice on medical treatments. She cannot guarantee a specific outcome as a result of working together. I assume full responsibility for any risks (or rewards!) from my use of her coaching services, and the decisions I make. I hereby forever release Susan A. Casserd and her agents and assigns from any liability.
This agreement is governed by the laws of King County, Washington, and is the entire agreement between us, and no other agreement or representations, verbal or otherwise, are of any force or effect.
Your agreement and purchase indicate full compliance with the requests above, and complete understanding of the services to be provided.
